Correct build errors when CONFIG_ENABLE_ALL_SIGNALS is not defined
- sched makefiles: Move pending-signal helpers from the ENABLE_ALL_SIGNALS-only
list to the !DISABLE_ALL_SIGNALS list so signal dispatch is available in
PARTIAL builds sched: make SIG_PREALLOC_ACTIONS, SIG_ALLOC_ACTIONS and
SIG_DEFAULT depend on ENABLE_ALL_SIGNALS
- sched: fix ifdefs around pending-signal queue access and signal-mask for
PARTIAL/DISABLE modes
- arch: gate SYS_signal_handler / _return calls and SYSCALL_LOOKUP(signal)
with ENABLE_ALL_SIGNALS

debug.h is a NuttX-specific, non-POSIX header. Placing it in the
top-level include/ directory creates naming conflicts with external
projects that define their own debug.h.
This commit moves the canonical header to include/nuttx/debug.h,
following the NuttX convention for non-POSIX/non-standard headers,
and updates all in-tree references.
A backward-compatibility shim is left at include/debug.h that
emits a deprecation #warning and re-includes <nuttx/debug.h>,
allowing out-of-tree code to continue building while migrating.

Signals in NuttX serve two primary purposes:
1. Synchronization and wake-up:
Signals can be used to block threads on specific signal sets and later
wake them up by delivering the corresponding signals to those threads.
2. Asynchronous notification:
Signals can also be used to install callback handlers for specific signals, allowing threads to
asynchronously invoke those handlers when the signals are delivered.
This change introduces the ability to disable all signal functionality to reduce footprint for NuttX.

The TCB initializes the pid early, but the argvstack is not initialized
at that time. This may cause invalid addresses to be obtained when
nxsched_get_stackargs is called during task enumeration.
Initialize the argv pointer to NULL to enable safe validity checks.

Kthreads can share the group data so that to reduce overheads.
This implements shared kthread group via:
- use `tcb_s` instead of `task_tcb_s` for kthreads
- use `g_kthread_group` when creating kthreads
- use stackargs to start tasks and kthreads

This moves task / thread cancel point logic from the NuttX kernel into
libc, while the data needed by the cancel point logic is moved to TLS.
The change is an enabler to move user-space APIs to libc as well, for
a coherent user/kernel separation.
argv is allocated from stack and then belong to userspace,
so task_info_s is a best location to hold this information.

since the standard require the caller pass the name explicitly
https://pubs.opengroup.org/onlinepubs/009695399/functions/posix_spawn.html:
The argument argv is an array of character pointers to null-terminated strings.
The last member of this array shall be a null pointer and is not counted in argc.
These strings constitute the argument list available to the new process image.
The value in argv[0] should point to a filename that is associated with the
process image being started by the posix_spawn() or posix_spawnp() function.

it is wrong to define a new grpid_t, but not reuse pid_t,
because it make getpid(parent) == getppid(child) impossible.

Summary:
- During reviewing sched_lock() in nxtask_assign_pid(),
I noticed that g_pidhash is not protected by a critical section
- Because g_pidhash is accessed in an interrupt context,
it should be protected by a critical section.
- Actually, nxsched_foreach(), nxsched_get_tcb() and so on
use a critical section.

-Move task_init() and task_activate() prototypes from include/sched.h to include/nuttx/sched.h. These are internal OS functions and should not be exposed to the user.
-Remove references to task_init() and task_activate() from the User Manual.
-Rename task_init() to nxtask_init() since since it is an OS internal function
-Rename task_activate() to nxtask_activate since it is an OS internal function
Move the prototype for the internal OS from from sched/sched/sched.h to include/nuttx/sched.h. This was done because binfmt/binfmt/excecmodule.c requires the prototype for sched_releasetcb() and was illegally including sched/sched/sched.h. That is a blatant violation of the OS modular design and the person that did this should be hung up by their thumbs. Oh... I did that back in a bad moment in 2014. Now that is made right.

1. Add semaphore uninterruptible wait function
2 .Replace semaphore wait loop with a single uninterruptible wait
3. Replace all sem_xxx to nxsem_xxx
* Unify the void cast usage
1. Remove void cast for function because many place ignore the returned value witout cast
2. Replace void cast for variable with UNUSED macro
